The first year, by caseload
Nothing here fails on a date. It fails at a volume — which is why practices are routinely caught out by it, having felt fine the month before.
Everything still fits in your head, so nothing gets written down
This is the stage that feels fine and quietly sets the pattern. Notes get written from memory at the end of the day, the cancellation policy lives in your inbox rather than a signed form, and there is no standard for what a good note contains because there has been no need for one. None of it hurts yet. All of it becomes the practice's default.
Scheduling stops being free
Rescheduling becomes a phone-tag problem, no-shows start costing real money, and the gaps left by cancellations no longer fill themselves. This is usually the first point where the admin load is visibly eating clinical time — and the first point where automated reminders and self-rescheduling pay for the system outright.
Billing becomes the bottleneck
Claims stop going out weekly because there is no time to do them weekly. Denials accumulate faster than they get worked, and because nothing is tracking days in accounts receivable, the practice discovers the problem as a cash-flow squeeze rather than as a number. Practices that reach this point without claim tracking usually write off revenue they had already earned.
You need to see the practice, not just the calendar
Utilization, no-show rate, denial rate and client retention stop being curiosities and start being the things that decide whether hiring is affordable. A second clinician also introduces access control, caseload distribution and supervision — the point at which a solo setup genuinely stops fitting.
The one worth doing early
Documentation habits. Everything else on this list can be fixed by switching systems later, at the cost of a migration. Documentation cannot: notes written thinly in year one are still thin when a payer audits them in year three, and there is no retroactive fix that is also honest. Getting the templates and the standard right while the caseload is small is the cheapest quality decision available to a new practice.
